Samaritan Health Services Employees
Live Well Studio, LLC classes are eligible for your Wellness Dollars. You have a total of $300 Wellness Dollars to use each year. You may credit any part of that toward classes at Live Well Studio, LLC. We will create an account in our system for you and you can use your Wellness Dollars for group mat classes, pilates equipment classes, zumba, or workshops . Like any credit purchased at the studio, the expiration date on your account is 365 days from the date the account is created.
To Bill Samaritan Health Services and set up your Live Well Studio, LLC account we will need the following information from you.
For SHS billing:
- Employee Name
- Employee Number
- Location of Employment
- Contact Phone Number (for SHS)
For Live Well Studio LLc account:
- Email Address
- Mailing Address
- Phone Number (if different)
Live Well Studio LLC sends a bill to SHS about twice per month. You are welcome to begin attending classes before we are paid by SHS. However, if your benefit is denied due to insufficient funds, or for any other reason, then you will be personally responsible for the unpaid balance.
